Fuse: Engine Coolant Fan

I have a 2004 Mazda 3 2.3L. I'm trying to locate the engine coolant fan fuse, the only fan fuse I can locate is labeled "fan 40A". Is this the correct fuse?
- how can I tell if this fuse is shot? it doesn't look like the standard fuse you just pull out.
- is there any other fan fuses to check that aren't labeled "fan", there's a few other "Engine" fuses.

My car is over heating, and the fan isn't kicking on.

The coolant reservoir has a crack on the bottom, it will get hot & start to boil and then the coolant streams out from the bottom of the reservoir.

My AC pulley snapped, so I can't turn on my AC to see if that fan kicks on.
