Bad Idle...need Help

my idle stays at around 1500-2000 and doesn't kick down even when warm... however it does drop down to about 1000 after i redline it real quick........I have tried to adjust the screw on the throttle body but with no success...
Does anybody have a remedy for this....
I was told i might have a vacum leak somewhere... would this cause this to happen???
