Has anyone experienced a issue with the telescoping part of the wheel sliding in when you apply the brakes?

 

Gotta ask; you're pushing the release arm all the way back up into the bottom of the column when you adjust the wheel? If you're sure you are doing everything right, take it into the dealer to get repaired, I would consider it a safety issue.
